How to Report a Positive Case

If your child tests positive for COVID-19, or is exposed to a positive case (6 feet or less for 15 minutes or more) it is important to contact your school nurse as soon as possible using the form linked below. The District will provide communication updates in a timely manner while respecting the privacy of our students and staff.

TO NOTIFY THE DISTRICT THAT YOUR STUDENT HAS BEEN EXPOSED, HAS SYMPTOMS, OR HAS TESTED POSITIVE FOR COVID-19,  COMPLETE THIS FORM.

The ACHD has worked to develop this very useful app that assists in determining whether or not one should quarantine and for how long.
